Starting from the 2026/2027 winter semester, the following applications must be submitted exclusively via the USOSweb system:

1. Application for an electronic student ID card (ELS)
2. Application for a duplicate electronic student ID card (ELS)
3. Application for course recognition/credit transfer (inclusion of course(s) in academic record)- in accordance with the procedure
4. Application to pay fees in installments
5. Application for a refund of overpayment
6. Application for transfer to another faculty
7. Application for leave for a pregnant student
8. Application for Dean’s leave
9. Application for short-term leave from classes
10. Application for leave for a student who is a parent
11. Application for professional leave
12. Application for medical leave (with attachment)

Starting from the 2026/2027 winter semester, the following applications must be submitted exclusively via the EDOK system:

1. Resignation from studies
2. Request for reconsideration of the case – appeal

Other applications submitted to the Dean's Office:

1. Transfer to another university - application to download. Transfers are made during the inter-semester break.
   The student submits a signed application to the dean's office, which is reviewed by the Dean.
2. Application to the Dean- application to download
3. Application for replacement subjects- application to download
4. Application for the creation of a retake subject- application to download
5. Application to repeat a stage of studies- application to download
6. Realization subjects in the form of a 15-week calendar of classes for students of the diploma semester- application to download
7. Application for the fourth and another implementation of a subject- application to download
8. Realization of subject in externalistic way (without attending classes)- application to download
